Services

Village Health Medical Practice is a member of the Partnership Health PHO this enables us to offer a wide range of quality services for enrolled patients, some of these services include:

  • B4 school checks for 4-5yr olds

  • Childhood Immunisations

  • Sexual Health consultations for those under 21yrs

  • Maternity Care

  • Diabetes Review Checks

 

For some patients the Care Plus and Access programmes may allow for reduced doctor's fees.

If you wish to enroll with Village Health Medical Practice please call in to the surgery and complete an Enrolment form. Enrolment is open to all permanent residents of NZ and those that have a work visa for two years continuous stay in New Zealand. Details of further entitlement can be obtained from www.moh.govt.nz

Repeat Prescription Policy

​

Patients may be able to receive a repeat prescription without attending a doctor’s appointment. This is ideally discussed with your doctor during a consultation. These circumstances include but are not limited to: Prescription item has been prescribed at this practice before, patient is stable on this medication with no monitoring currently required, prescribing doctor is agreeable to prescribing the item with no consultation.

 

When requesting a repeat prescription by email, fax or phone, please supply the following:

  • Your Name

  • Date of Birth

  • Your phone number

  • Your Doctor’s Name

  • The medications you require

  • The Pharmacy you would like it faxed to or if you would prefer to collect it from the surgery.

 

We endeavour to do our best to organise repeat prescriptions as soon as possible. We do ask that you allow at least three days for processing repeat prescriptions. If you require a repeat prescription urgently, please speak to the Practice Nurse directly. Do not leave a message on our answerphone.
