Requirements for Enrolment

New Zealand Citizen
Please submit a copy with your enrolment form.

Work Visa Holder
Please submit a copy with your enrolment form. Visas with a minimum of 24 months for eligibility.

Utility Bill with current Home Address
A copy of a recent bill with your current home address may be requested during the enrolment process.

Permanent New Zealand Resident
Please submit a copy with your enrolment form.

Proof of Identity
Photo ID is required for enrolment to be considered. A Child's enrolment requires a copy of their Birth Certificate

Not Currently Enrolled with a Healthcare Provider In the area

New to the area with no current Healthcare Provider

Monday & Thursday: 8am to 5pm
Tuesday & Wednesday**: 8am to 6pm
Friday: 8am to 4pm
Saturday & Sunday: Closed
** Please note on Wednesdays between 12pm and 2pm our staff will be unavailable due to training and professional development**
